I am an in-network provider for Aetna, Cigna, Regence/Blue Cross Blue Shield, Optum, Providence, United Behavioral Health, MHN, Pacific Source, and Reliant Behavioral Health (RBH).
For other insurance companies, I am an out-of-network provider. I will provide you with a statement with all the information necessary to submit to your insurance company for reimbursement. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- What is the coverage amount per therapy session for an out-of network provider?
- Do sessions need to be preauthorized?
Please remember that you, not the insurance company, are ultimately responsible for paying the agreed upon fees.
For those who cannot afford the full fee, please inquire about sliding fee options as I reserve a limited number of slots for reduced rate.
Payment are due at the time of service and may be made by cash, check and all major credit cards. If using insurance, all co-payments and co-insurance are expected at the time of service.
I ask that you kindly give 24 hour notice if you need to change or cancel your appointment. If you do not show up for your scheduled therapy appointment, and have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session.
